Service Description

Permit: If you applying for a Permit or ID then you will need to: • Bring a form of identification • Review / complete the applicable form Drivers Permit (Click on Pencil to the right to access forms -->)

Cancellation Policy

To cancel or reschedule, you must do so at least 24 hours prior to your scheduled appoint time to avoid a potential rescheduling fee of $20.00. Email appointments@susitnadmv.com with your appointment date/time and request to cancel or call 907-733-2266 and leave your appointment date/time in a voice message.
